All our patients need to be registered for a PATCHS account with their email address and a password (or be linked to the account of someone who cares for them).
Registration with PATCHS is straight forward – you just need to complete a simple form with your details to allow us to match you with our records. Then you need to choose a password – you can use the same one you used for askmyGP if you like.
You can make a request for you or someone you care for immediately after registering.
Note that if PATCHS is closed to clinical requests, you are still able to register.
